Case Management Referral Form Procedures

 

  • Worker will send completed/unsigned Case Management Referral Form via an alert to the Case Manager (make sure to include reason for referral)
  • The Case Manager will pursue the request and respond to the requesting worker if she/he needs more information as well as to inform them of the outcome of the referral.
  • Note: there will be a separate entry for each communication between the two parties
  • The Case Management Referral Form will be located in the Communication Between Staff section of the chart as well as the Service Entry
  • Once the Case Manager has finalized the referral, she/he will send final alert to the referring worker 
  • Referring worker will then e-sign the event
